HealthCollaborate™ is designed to enable clinicians globally to form/join communities of providers and to securely communicate, exchange patient data, collaborate on patient cases, share ideas, trade documents, send and receive messages containing PHI such as referrals, images and results, create discussion forums, share knowledge, ask questions and more. Patients can receive health education, appointment and medication reminders, transition of care, visit summaries and other Health Information documents directly from providers. Patients can also form own community of patients, create discussion forums, send medical records to physicians and more.
